During custom form design, this program enables you to define a lookup list of possible entries for a specific custom form field. The program is accessed from the Field Properties pane of the Custom Forms Entry program (click on the Edit hyperlink for the Configure field within the Field validation section).
During custom form entry, you can view the list of items and select the required entry from the list.
